Accident/Injury Reporting

To report an accident or injury, please submit all work-related incident reports online via the WSU Incident Report Form found on the Environmental Health & Safety website (S25.20.8).

Supervisors are required to fill out a Supervisor's Incident Report Form and submit it to Kevin Crowley. (S25.25.5)
